This Operator suffered a complete breakdown in their compliance regime following the breaching of Undertakings given at a previous Public Inquiry. Forward-planning was non-existent; record-keeping was sparse and intermittent; and drivers were regularly pulling driver-cards. There was also a pressing Financial Repute issue to be resolved.
